IMPLEMENTATION

THE NAV-CARE PROGRAM IS IMPLEMENTED THROUGH COMMUNITY-BASED ORGANIZATIONS WITH TRAINED VOLUNTEERS. 

Step 1

Communities who choose to implement Nav-CARE use the Implementation Manual to work through a series of questions to determine whether Nav-CARE is a good fit with their organization. 


Step 2

If Nav-CARE is a good fit, the organization then contacts the Nav-CARE team to discuss onboarding and sign a memorandum of agreement.

The Nav-CARE team then provides the toolkit including:

Facilitators Guide

Learning Manual


Step 4

The organization recruits experienced volunteers and mentors volunteer navigation training. These specially-trained volunteers then work with one or two people with declining health. The volunteer’s goal is to provide long term navigation support and companionship. 


Funding Opportunity

With a contribution from Health Canada, we are inviting hospice palliative care organizations to apply for 2-year funding to implement Nav-CARE in their site and to support implementation in additional hospice palliative care sites. 

The 2022-23 funding cycle is now closed. The next funding cycle will be announced on October 1, 2022
